Do you want to make a difference to the health and wellbeing of your community?
We are looking for passionate and driven individuals to join our Talk Community Healthy Lifestyle Team. The service provides integrated behaviour support for smoking cessation, weight management, alcohol reduction, physical activity and falls prevention, all embedded in sustainable behaviour change and positive wellbeing.
As part of a new exiting opportunity the post holder will work within Wye Valley Hospital trust and Herefordshire and Worcestershire Health & Care Trust to deliver an inpatient stop smoking treatment programme. 
The post holder will 
Work with hospital admissions teams to support the identification of patients admitted to hospital who smoke 
Offer a personalised tobacco treatment programme, commencing during their inpatient stay and continuing post discharge with access to nicotine replacement therapy and behavioural support 
Work with the community element of the service and other partner organisations to ensure the delivery of stop smoking support post discharge 
Work with staff teams across WVT to embed the long term commitment of the inpatient smoking cessation treatment programme 
The ideal candidates will be motivated, organised and proactive advisors, with experience of working within a healthcare environment and delivering holistic community health programmes, passionate about promoting healthy lifestyles and delivering motivational change which is client centred.
The successful candidate will also be confident and experienced in delivering support to groups and 1-2-1 meetings.
We are looking for individuals with strong communication skills, who are confident delivering holistic health and lifestyle advice to individuals and groups, including carrying out initial assessments and brief interventions using proven behaviour change techniques, being able to adapt and change their delivery techniques to suit vulnerable individuals to develop increase and strengthen their skills to lead a healthier lifestyle. 
A background in smoking cessation, nutrition, exercise, public health, community health, recovery or support working and experience of working within a health care environment would be beneficial along with being excellent team player and having a willingness to learn and develop.
