The Role

We are looking for a Family Support Worker to help us on our improvement journey.

Working within the Children (and young people) with Disabilities team, you will thrive bringing your knowledge and experience to this varied, supportive and fast-paced team. 

You will work within a team made up of social workers and family support workers, who support children and young people, alongside their families. We are a 0-25 service and your role will also include the Personal Advisor role for a small cohort of young people who are leaving care.

You will be responsible for allocated children and will review their support plans, inclusive of reviewing short breaks services to ensure that this is appropriate and supporting the children you work with to access the world around them.

You will work with families and social workers to develop support plans and undertake work with families to support them to achieve goals for their child and family. 

You will be Level 4 (or above) qualified in a health, education, or social care field with a minimum of 12 months experience working with vulnerable families. You will need to be confident in working in complex environments and a flexible approach to a demanding role, you will be dedicated to improving the lives of children and families within Herefordshire.
