Assessment and Review Officer
Salary: £28.971| Permanent | Enhanced DBS | Full time
We’re looking for a caring, proactive and skilled Assessment and Review Officer to join our Home First team. You’ll play a key role in helping adults regain confidence and independence after illness, injury or a period of reduced ability — supporting them to achieve their personal goals and live fulfilling lives in their own homes.
About the Role
As part of our Reablement Service, you’ll:
- Complete timely assessments, reviews and support plans that reflect each person’s needs, goals and strengths.
- Coach and empower people to take an active role in their own recovery, using a reablement approach that builds confidence and independence.
- Assess and manage risk, ensuring safe, person-centred care.
- Work closely with colleagues, families, carers and partner agencies to deliver consistent and effective support.
- Support the deployment of care staff and contribute to the day-to-day delivery of a high-quality, CQC-compliant service.
- Teach and demonstrate the use of mobility aids and assistive technology.
- Maintain accurate, up-to-date electronic records and documentation.

About You
You’ll be an experienced care professional with:
- Experience of assessing adult care needs, risk management and care planning.
- A background in social care, health care or community settings.
- Excellent communication, problem-solving and decision-making skills.
- The ability to work flexibly — independently and as part of a team — to meet service needs 7 days a week.
- A full driving licence and access to your own vehicle.
You’ll also be self-motivated, compassionate, and confident in supporting vulnerable adults while maintaining the highest standards of safeguarding and professionalism.
Qualifications:
- Diploma/NVQ Level 3 in Health or Social Care (or willingness to work towards).
- Competency certificates in care, manual handling, and simple aids/adaptations (or willingness to obtain).
