Introduction

Technology Enabled Care (TEC) can help people remain independent, safe and connected by using the right technology around their strengths, routines and outcomes. Herefordshire Council is continuing to develop its TECS service from a traditional, reactive telecare offer into a more proactive, preventative and evidence-led service across Adult Social Care and health pathways.
The Role

We are looking for an experienced, practical, person-centred and digitally confident practitioner to join our Technology Enabled Care Service.
This is a hands-on assessment and installation role combining frontline delivery with service development. Applicants must already have practical experience of independently installing, configuring, testing and demonstrating a wide range of telecare and Technology Enabled Care (TEC) equipment in people’s homes. This must include experience of both traditional telecare products and wider proactive technology, including daily activity monitoring systems. You will assess people over the phone, in their own homes and in other community settings, identify appropriate solutions and review whether the technology is achieving the agreed outcomes. You will also help us extend our ‘assess and install’ model and use remote and proactive monitoring to inform care and support decisions.
What will you be doing?

You will manage referrals and complete strengths-based TEC assessments, working with adults, carers and professionals to understand what matters to the person, the risks they face and the outcomes they want to achieve. You will independently select, configure, install, pair, test, fault-find and explain a broad range of telecare, assistive technology, environmental safety and remote monitoring solutions. This will include linked and standalone alarms, personal triggers and falls technology, property and environmental sensors, occupancy and exit monitoring, and daily activity monitoring systems.
You will set up and review daily activity monitoring, interpret the information produced and present clear evidence of changes in routines, risks or needs. You will record outcomes such as improved independence and safety, support for carers, reduced or avoided care, prevention of escalation and support for hospital discharge or reablement. You will also contribute to equipment evaluation, pilot projects, training, stock oversight and continued improvement of the service.
About You

Practical telecare and TEC installation experience is an essential requirement for this role. You must be able to demonstrate recent hands-on experience of installing equipment safely in people’s homes across a wide range of products, including daily activity monitoring. You must also understand how technology can support person-centred outcomes rather than simply providing equipment. You will be:
- Experienced in assessing needs, risks and desired outcomes within a health, social care, housing, reablement or community setting.
- Able to demonstrate proven experience of independently installing, configuring, testing, fault-finding and demonstrating a wide range of telecare and TEC products, including daily activity monitoring systems.
- Experienced in interpreting daily activity and other monitoring information, identifying concerns or trends and communicating findings clearly.
- A strong communicator who can build trust with adults, carers, colleagues, providers and partner organisations.
- Organised, able to manage a varied field-based caseload and comfortable working independently as well as within a multidisciplinary team.
- Committed to safeguarding, dignity, choice, equality and the responsible use of personal information.
