Introduction

The Care Leavers Service within Herefordshire Council plays a vital role in supporting young people as they move from the care system into independent adulthood. The team works alongside each young person to provide practical guidance, emotional support, and clear pathways into safe housing, education, training, employment, and positive health and wellbeing. By building trusting relationships and ensuring that every care leaver has a reliable point of contact, the service helps young people feel confident, empowered, and prepared for the future.
The service is structured into three dedicated teams, each with its own focus but working closely together to offer a seamless and consistent experience. Whether it’s providing personal advisor support, coordinating practical resources, or helping young people navigate the challenges of independence, the combined work of these teams ensures every care leaver receives the right support at the right time. The aim is to give young people the best possible start to adulthood, with a strong foundation, meaningful opportunities, and a network of professionals who believe in their potential.
